Medicare for 2027: fall enrollment and the new Part D cost limits

Medicare uses a different enrollment calendar from the ACA Marketplace. Review the 2027 drug-cost changes with your exact medications and pharmacies.

Mark the Medicare dates, not the Marketplace dates

Medicare Open Enrollment runs October 15 through December 7, with changes generally effective January 1 of the following year. This is the fall opportunity to review Medicare Advantage and Part D choices. It is not the same as initial enrollment in Medicare, and it is not the ACA Marketplace enrollment period. Part D’s annual out-of-pocket threshold changes

Medicare.gov lists the covered Part D out-of-pocket threshold as $2,100 in 2026 and $2,400 in 2027. The maximum Part D deductible rises from $615 to $700, although some plans have lower or no deductibles. These numbers concern covered Part D drug spending; they are not a cap on all Medicare spending or on every prescription purchased outside coverage. Medicare.gov — Part D costs in 2026 and 2027.

Run the comparison with your actual drug list

Enter the current medicine name, dosage, frequency and preferred pharmacies in Medicare’s plan comparison tool. Review the estimated yearly cost as well as the monthly premium. A low-premium plan can still be less suitable if its drug list or pharmacy pricing is a poor match. Medicare.gov’s comparison tool now provides a 2027 coverage-year option. Check help with drug expenses

Medicare’s Extra Help program can assist eligible people with Part D premiums and cost sharing. Medicare.gov publishes 2027 details and explains that people can reapply if their income or resources change. Do not decide eligibility from a generic news headline or assume a prior denial applies forever. Use the official application process and current rules. Medicare.gov — Extra Help with drug costs.

Look at medical coverage alongside prescriptions

If you are reviewing a Medicare Advantage option, verify the providers, hospitals, service area and medical cost-sharing terms as well as its drug coverage. If you are considering returning to Original Medicare, investigate drug coverage and any Medigap rights before making the change. The fall enrollment period does not mean unrestricted access to any Medigap policy; Medicare.gov explains the limits. Medicare.gov — Medicare Open Enrollment.

Make a comparison that another person can follow

Write the current plan in one column and each serious alternative in another. Record the annual drug estimate, preferred pharmacy, key providers and anything unresolved. If a relative is helping, preserve the member’s choices and consent rather than treating an online comparison as permission to enroll. Keep identity and medication information in the official or otherwise appropriate secure process.

  • Review the plan’s annual change notice.
  • Select 2027 in the comparison tool.
  • Update medications and pharmacies before comparing prices.
  • Check medical access and drug costs separately.
  • Confirm the effective date and save the enrollment record.

Use official support for Medicare-specific questions
